Do you have a complaint about a dentist?
The Chicago Dental Society works with the Illinois State Dental Society
to maintain a Mediation and Peer Review Committee to help the public and
the profession amicably settle disputes or misunderstandings. To begin
the process, download and complete the Request for Review of
Dental Services form. All complaints must be submitted in writing.
